First Dental Visit – Your Initial Consultation with Dr. Miller
Your appointment usually will consist of a thorough dental examination and review of your treatment options. If necessary, treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require further evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Any x-rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.

Dentist Financial Policy and Payment Options
For your convenience, we accept:
- Cash
- Personal Checks
- Major Credit Cards
- HSA or FSA
Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
We also offer convenient online payment for our established patients.
Insurance
As a service to our patients, our practice accepts most dental insurance programs, including non-managed care and PPO out-of-network. We are not part of any managed care network. Our accounting staff will prepare all the necessary forms for your dental benefit claims.
However, we respectfully remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be.
The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies take care of claims promptly while others delay payment for several months.
